The shipper bears sole responsibility for the document's accuracy and completeness, must sign it personally, and ensure it travels with the shipment. Carriers and freight forwarders are obliged to refuse cargo presented without a correct declaration; accepting a shipment with known errors creates shared liability. Errors in classification or missing information can result in fines, transport bans, and civil liability claims. For air freight, the IATA-DGR prescribes the applicable form; for sea transport, the Multimodal Dangerous Goods Form under the IMDG Code applies.
